Vildagliptin is a DPP-4 inhibitor that increases incretin hormones (GLP-1, GIP), leading to increased insulin release and reduced glucagon after meals. Tablet for oral use. Take exactly as prescribed, usually with meals to reduce stomach upset. Swallow whole with water; do not crush/chew if it is an SR/ER tablet. Maintain diet/exercise and monitor blood sugar regularly.

Not for type 1 diabetes or diabetic ketoacidosis. Metformin may rarely cause lactic acidosis—risk increases with kidney disease, dehydration, severe infection, heart/lung failure, heavy alcohol intake, or older age; stop and seek care if rapid breathing, severe weakness, abdominal pain, or unusual drowsiness occur. Check renal function before and during therapy; avoid/use with caution in moderate-to-severe renal impairment. Vildagliptin: monitor liver enzymes; discontinue if jaundice or persistent elevated transaminases occur. Temporarily stop metformin around iodinated contrast studies or major surgery as advised by doctor. Use caution in pancreatitis history; stop if severe persistent abdominal pain. Long-term metformin may cause vitamin B12 deficiency—periodic monitoring may be needed.
Alcohol (increases lactic acidosis risk), iodinated contrast media, carbonic anhydrase inhibitors (e.g., topiramate, acetazolamide), cimetidine, diuretics, corticosteroids, thyroid hormones, sympathomimetics, beta-agonists, antipsychotics, oral contraceptives, ACE inhibitors (may enhance glucose-lowering), insulin and sulfonylureas (increased hypoglycaemia risk); drugs affecting renal function may increase metformin levels
