Important Warnings

Use with caution in liver disease; long-term PPI use may increase risk of vitamin B12 deficiency, low magnesium, fractures and infections (e.g., C. difficile). Domperidone may cause QT prolongation/serious heart rhythm problems—avoid in known QT prolongation, significant cardiac disease, or with other QT-prolonging medicines; seek care for palpitations or fainting. Not recommended in severe hepatic impairment; use under medical supervision in elderly. Stop and consult doctor if black stools, vomiting blood, or persistent/worsening symptoms.

Domperidone: strong CYP3A4 inhibitors (ketoconazole/itraconazole
clarithromycin/erythromycin
ritonavir) and other QT-prolonging drugs (amiodarone
sotalol
certain antipsychotics/fluoroquinolones) increase arrhythmia risk. Pantoprazole: may reduce absorption of drugs needing acidic pH (ketoconazole
atazanavir); may affect warfarin/INR (monitor)
high-dose methotrexate (avoid/monitor)
digoxin (with hypomagnesemia).
