Glimepiride 2 mg + Metformin Hydrochloride 750 mg (modified release)
Type 2 diabetes mellitus: to control high blood sugar in adults when diet and exercise alone are not sufficient; helps prevent diabetes-related complications.
Glimepiride (a sulfonylurea) increases insulin release from the pancreas. Metformin (a biguanide) reduces glucose production in the liver and improves insulin sensitivity, decreasing intestinal glucose absorption. Together they lower fasting and post-meal blood glucose.
Tablet (modified release): take exactly as prescribed, usually once daily with or just after food (preferably breakfast). Swallow whole; do not crush or chew. Maintain diet/exercise and monitor blood sugar; if a dose is missed, take the next dose at the usual time (do not double).

Schedule H: use on prescription only. Risk of hypoglycaemia (especially with missed meals, excess exercise, alcohol, or with other antidiabetics); carry sugar/glucose. Metformin may rarely cause lactic acidosis—risk increases with kidney disease, dehydration, severe infection, shock, alcohol use, or hypoxia; stop and seek care if rapid breathing, severe weakness, abdominal pain, or drowsiness occur. Avoid/adjust in renal impairment; monitor eGFR, liver function, and vitamin B12 with long-term use. Temporarily stop metformin before/at time of iodinated contrast procedures or major surgery and restart only when kidney function is stable. Use caution in elderly and in people with G6PD deficiency (rare haemolysis with sulfonylureas).
