Alcohol increases risk of metformin‑associated lactic acidosis and potentiates hypoglycaemia; iodinated contrast media can precipitate acute kidney injury leading to metformin accumulation/lactic acidosis; insulin or other antidiabetics (e.g.
linagliptin
empagliflozin) additively increase hypoglycaemia risk; beta‑blockers (e.g.
propranolol) mask adrenergic warning signs of hypoglycaemia; fluoroquinolones (e.g.
levofloxacin) may cause dysglycaemia; corticosteroids/diuretics (prednisolone
hydrochlorothiazide) raise glucose and reduce efficacy; cimetidine reduces metformin renal clearance via OCT2/MATE inhibition increasing exposure; ACE inhibitors (e.g.
enalapril) may enhance glucose‑lowering causing hypoglycaemia in some patients; NSAIDs may increase sulfonylurea effect and hypoglycaemia.
