Hyperkalemia, nausea, vomiting, diarrhea, abdominal cramps, dizziness, headache, fatigue, gynecomastia, breast tenderness, menstrual irregularities, decreased libido, rash

Risk of high potassium (hyperkalemia), especially in kidney disease, diabetes, dehydration, or when combined with ACE inhibitors/ARBs or potassium supplements. Avoid in severe kidney impairment, anuria, Addison’s disease, or known hyperkalemia. Monitor serum potassium, sodium and kidney function regularly. May cause dizziness; use caution while driving. Use caution in liver disease.

ACE inhibitors (e.g., enalapril), ARBs (e.g., losartan), aliskiren, potassium supplements/salt substitutes, other potassium-sparing diuretics (amiloride, triamterene), NSAIDs (ibuprofen, diclofenac), lithium, digoxin, trimethoprim/cotrimoxazole, heparin/LMWH
