Treatment of erectile dysfunction (impotence) in adult men; helps achieve and maintain an erection with sexual stimulation

Sildenafil is a phosphodiesterase-5 (PDE5) inhibitor. It increases cGMP levels in penile tissue, causing relaxation of smooth muscles and increased blood flow during sexual stimulation.

Oral tablet. Take as advised by doctor, usually 30–60 minutes before sexual activity (can take up to 4 hours before). Swallow whole with water. Avoid heavy/fatty meals as they may delay effect. Do not take more than once in 24 hours.

Headache, flushing, dizziness, nasal congestion, indigestion, nausea, blurred vision, sensitivity to light, back pain

Not for women/children. Do not use with nitrates (e.g., nitroglycerin, isosorbide) due to dangerous fall in blood pressure. Use with caution in heart disease, recent heart attack/stroke, uncontrolled hypertension/hypotension, severe liver impairment, severe kidney disease, retinitis pigmentosa, bleeding disorders, active peptic ulcer. Seek urgent care for chest pain, sudden vision/hearing loss, or erection lasting >4 hours (priapism).

Nitrates/NO donors (contraindicated), riociguat (contraindicated), alpha-blockers (e.g., tamsulosin/doxazosin—may cause hypotension), strong CYP3A4 inhibitors (ketoconazole, itraconazole, ritonavir, clarithromycin—raise sildenafil levels), CYP3A4 inducers (rifampicin—reduce effect), antihypertensives, grapefruit juice (may increase levels)
