Management of moderate to severe chronic pain requiring around-the-clock opioid analgesic treatment when alternative treatments are inadequate

Tapentadol is a centrally acting analgesic with dual action: it is a mu-opioid receptor agonist and also inhibits norepinephrine reuptake. These actions reduce pain signal transmission and perception in the brain and spinal cord.

Oral extended-release tablet. Swallow whole with water; do not crush, chew, or split. Take exactly as prescribed at the same times each day; may be taken with or without food. Do not stop suddenly without medical advice.

Nausea, vomiting, constipation, dizziness, drowsiness, headache, dry mouth, sweating, fatigue

Opioid medicine: risk of addiction, abuse and misuse; can cause life-threatening respiratory depression, especially at initiation or dose increase. Avoid use with alcohol or other CNS depressants (sedatives, sleeping pills) due to excessive sedation/respiratory depression. Use with caution in elderly, liver or kidney impairment, head injury, seizure disorders, low blood pressure, respiratory disease (e.g., COPD, sleep apnea). May cause drowsiness—avoid driving/operating machinery. Risk of serotonin syndrome when combined with serotonergic drugs. Not for acute pain or as-needed use in ER form; keep out of reach of children.

Alcohol, benzodiazepines and other sedatives/CNS depressants, other opioids, MAO inhibitors (contraindicated/avoid), SSRIs/SNRIs/TCAs/triptans/linezolid (serotonin syndrome risk), antipsychotics, gabapentinoids, strong CYP enzyme inducers/inhibitors may alter effects (clinical monitoring advised)

Store below 25°C in a cool, dry place away from light and moisture. Keep in original pack and out of reach of children.
