Telmisartan is an angiotensin II receptor blocker (ARB) that relaxes blood vessels by blocking the action of angiotensin II. Amlodipine is a calcium channel blocker that reduces calcium entry into vascular smooth muscle, causing vasodilation and lowering blood pressure.
Take as prescribed by your doctor, usually once daily at the same time each day. Can be taken with or without food; swallow the tablet with water. Do not stop suddenly without medical advice; monitor blood pressure regularly.
Dizziness, headache, ankle swelling (edema), flushing, palpitations, fatigue, nausea
May cause dizziness—avoid driving or operating machinery if affected. ARBs like telmisartan can increase potassium; use caution with potassium supplements/salt substitutes. Monitor kidney function and blood pressure, especially in elderly and those with renal impairment or dehydration. Not recommended in pregnancy; can harm the fetus. Use caution in severe liver disease, heart failure, or aortic stenosis. Seek urgent care for facial/lip swelling (angioedema) though rare.
Potassium supplements/salt substitutes, potassium-sparing diuretics (e.g., spironolactone, eplerenone, amiloride), NSAIDs (e.g., ibuprofen, diclofenac) may reduce effect and worsen kidney function, Lithium (risk of toxicity), Other antihypertensives/diuretics (additive BP lowering), Aliskiren (avoid in diabetes/renal impairment), CYP3A4 inhibitors/inducers affecting amlodipine (e.g., clarithromycin, azole antifungals, rifampicin), Grapefruit juice (may increase amlodipine levels in some patients)
