Treatment of moderate to severe rheumatoid arthritis (RA) in adults; may also be prescribed for psoriatic arthritis and ulcerative colitis as per specialist advice and approved indications.
Tofacitinib is a Janus kinase (JAK) inhibitor. It blocks JAK enzymes involved in immune signaling, thereby reducing inflammatory cytokine activity and helping relieve joint pain, swelling and stiffness.
Take by mouth as prescribed by the doctor, usually once or twice daily. Can be taken with or without food; swallow whole with water. Do not stop abruptly; regular monitoring tests are required.
Upper respiratory tract infection, headache, diarrhoea, nausea, increased cholesterol, elevated liver enzymes, hypertension, rash
Prescription-only immunosuppressant. Increased risk of serious infections (TB, herpes zoster, opportunistic infections); screen for TB and hepatitis before starting. Risk of blood clots, major cardiovascular events and malignancy may be increased in certain patients—use only under specialist supervision. Monitor CBC, liver function and lipids; avoid use in severe hepatic impairment. Not recommended with other potent immunosuppressants/biologics. Seek urgent care for fever, persistent cough, breathlessness, chest pain, leg swelling, unusual bruising/bleeding.
Strong CYP3A4 inhibitors (ketoconazole, clarithromycin) may increase levels; strong CYP3A4 inducers (rifampicin, carbamazepine, phenytoin) may reduce effect; immunosuppressants/biologics (azathioprine, cyclosporine, tacrolimus, TNF inhibitors) increase infection risk; live vaccines should be avoided during therapy.
