Verify before you leave
Catching a missing field at the doctor's office takes 30 seconds. Catching it at RTO upload takes a fresh examination.
Privacy matters
Don't share your filled Form 1A on social media or messaging apps — it contains your full address, ID, and medical findings.
Digital and paper Form 1A look the same
An online-issued PDF should have all the same sections filled. The signature is digital but the rest is identical to a paper original.

Be cautious. Any 'sample filled Form 1A' you find online either has someone's real personal data (which they shouldn't have published) or is a generic template. Use this walkthrough to verify your own form rather than copying from a sample.
Missing doctor's signature, missing stamp, missing registration number, or a name mismatch with the Sarathi application. These are also the top reasons for RTO rejection at upload.
Either is acceptable, but it must be a valid registration number from a recognised state medical council or the National Medical Commission (formerly MCI). The number should be legible.
Most RTOs expect both — a clinic stamp/seal and the doctor's signature. A printed letterhead alone is usually not sufficient as it does not carry the doctor's certification weight.
Strikethrough corrections initialled by the doctor are usually accepted. Whitener is risky — it can be interpreted as tampering. If a section needs significant correction, ask for a fresh Form 1A.
