Where to Download Form 1A

Parivahan.gov.in (Primary Source)
The Ministry of Road Transport and Highways hosts all motor vehicle forms on the Parivahan portal. Navigate to the "Forms" or "Downloads" section and search for "Form 1A" or "Medical Certificate." The form is available in PDF format and can be downloaded and printed on A4 paper.
State Transport Department Websites
Individual state transport departments (e.g., Tamil Nadu Transport Department, Karnataka Transport Department) also host the form on their websites. The format is identical to the central version, though some states may include additional local instructions.
Sarathi Portal (sarathi.parivahan.gov.in)
The Sarathi online driving licence portal sometimes provides the form as part of the application process. When you start a DL application online, the portal may prompt you to download and get Form 1A filled before proceeding.
What the Blank Form 1A Contains
When you download Form 1A, you will see a single-page or two-page document (depending on the layout version) with these sections:
- Header: "Form 1A" with a reference to Rule 4 of CMVR, 1989. States "Medical Certificate" prominently.
- Applicant details section: Blank fields for name, age/date of birth, address, identification marks, DL number (if renewal), and vehicle class.
- Medical examination section: Blank fields for the doctor to record height, weight, vision (Snellen chart readings), colour vision (Ishihara test), hearing, limb assessment, and general physical and mental condition.
- Fitness declaration: A pre-printed statement where the doctor certifies the applicant as Fit / Fit with restrictions / Unfit, with space for the doctor's name, registration number, signature, date, and stamp.
Why Downloading the Blank Form Is Not Enough
Many people download the blank Form 1A expecting to fill it themselves and submit it to the RTO. This does not work. Here is why:
A doctor must examine you
Form 1A is a medical fitness certificate, not a self-declaration form (that is Form 1). A registered medical practitioner must conduct a physical examination — testing your vision, colour vision, hearing, limbs, and overall health — before filling out the medical section.
Doctor's signature and stamp are mandatory
The RTO verifies the doctor's medical council registration number, signature, and stamp on every Form 1A. A certificate without these elements is automatically rejected. The stamp must show the doctor's name and registration number clearly.
Self-filled forms are rejected
If the RTO detects that the medical section was filled by the applicant rather than a doctor, the application is rejected. RTOs cross-check doctor registration numbers and may call to verify. Submitting a forged medical certificate is a punishable offence under the Motor Vehicles Act.
