Important Warnings

Risk of hypoglycaemia (higher with missed meals, alcohol, heavy exercise or with other antidiabetics); metformin-associated lactic acidosis (seek urgent care for rapid breathing, severe weakness, abdominal pain); stop metformin temporarily before/after iodinated contrast procedures and during severe illness/dehydration; dapagliflozin may cause dehydration/low BP, ketoacidosis (can occur with normal sugars), serious genital infections; monitor renal function—use not recommended in severe kidney impairment; caution in elderly and those on diuretics; not for type 1 diabetes.

Drug Interactions

Insulin/other sulfonylureas (increased hypoglycaemia)
diuretics (increased dehydration/hypotension)
ACE inhibitors/ARBs (may enhance glucose lowering)
corticosteroids and sympathomimetics (raise glucose)
rifampicin (may reduce glimepiride effect)
azole antifungals/clarithromycin (may increase glimepiride effect)
alcohol (increases hypoglycaemia and lactic acidosis risk)
iodinated contrast media (lactic acidosis risk with metformin)
carbonic anhydrase inhibitors like topiramate (may increase acidosis risk)
