Dapagliflozin 5 mg + Glimepiride 2 mg + Metformin 500 mg (as Metformin HCl) per tablet
Type 2 diabetes mellitus: to improve blood glucose control in adults when diet and exercise alone or current therapy is inadequate.
Metformin decreases hepatic glucose production and improves insulin sensitivity. Glimepiride (a sulfonylurea) increases insulin release from pancreatic beta cells. Dapagliflozin (an SGLT2 inhibitor) increases urinary glucose excretion by reducing glucose reabsorption in the kidneys.
Oral tablet; take exactly as prescribed by the doctor, usually once daily with food (preferably with the first main meal) to reduce stomach upset. Swallow whole with water; do not crush/chew. Monitor blood glucose regularly.

Risk of hypoglycaemia (higher with missed meals, alcohol, heavy exercise or with other antidiabetics); metformin-associated lactic acidosis (seek urgent care for rapid breathing, severe weakness, abdominal pain); stop metformin temporarily before/after iodinated contrast procedures and during severe illness/dehydration; dapagliflozin may cause dehydration/low BP, ketoacidosis (can occur with normal sugars), serious genital infections; monitor renal function—use not recommended in severe kidney impairment; caution in elderly and those on diuretics; not for type 1 diabetes.
