Important Warnings

Do not self-medicate for anemia without evaluation (iron, B12, folate deficiency need confirmation). Use with caution in patients with history of hypersensitivity to B-vitamins/folate products. High-dose folate can mask vitamin B12 deficiency and allow neurological damage to progress—ensure B12 status is assessed when indicated. Patients with seizure disorders on antiepileptics should consult a doctor.

methotrexate (dose-dependent; folate may reduce toxicity)
trimethoprim/pyrimethamine
sulfasalazine
chloramphenicol (may blunt hematologic response)
metformin and PPIs (can reduce vitamin B12 levels if present in regimen)
