Important Warnings

Use with caution in severe liver disease. Long-term use may increase risk of low magnesium, vitamin B12 deficiency, bone fractures, and Clostridioides difficile-associated diarrhea; consult doctor if persistent diarrhea occurs. May mask symptoms of gastric cancer—seek medical advice for alarm symptoms (unexplained weight loss, vomiting blood, black stools, difficulty swallowing). If used for long durations, periodic monitoring may be needed.

methotrexate (high-dose; increased levels)
digoxin (risk with low magnesium)
diuretics (may worsen hypomagnesemia)

atazanavir/nelfinavir (reduced effect)
clopidogrel (generally less interaction than omeprazole but caution)
