Important Warnings

May cause dizziness and sleepiness—avoid driving/operating machinery until you know the effect. Use caution in elderly and in kidney disease (dose adjustment needed for pregabalin). Nortriptyline can cause constipation, urinary retention, and may worsen glaucoma/prostatic enlargement; caution in heart disease/arrhythmias. Do not stop suddenly (withdrawal symptoms). Monitor for mood changes/suicidal thoughts. Seek urgent care for facial swelling, severe rash, breathing difficulty (allergy/angioedema).

Alcohol and other CNS depressants (opioids
benzodiazepines
sedating antihistamines) increase drowsiness/respiratory depression risk; MAO inhibitors (contraindicated with nortriptyline); SSRIs/SNRIs and other serotonergic drugs (serotonin syndrome risk); other tricyclics/anticholinergics (additive side effects); QT-prolonging drugs (arrhythmia risk); strong CYP2D6 inhibitors (e.g.
fluoxetine
paroxetine
quinidine) may increase nortriptyline levels.
