Important Warnings

Not recommended in pregnancy (can harm fetus); stop and consult doctor if pregnancy occurs. Use caution in kidney disease, renal artery stenosis, dehydration/diuretic use, heart failure, liver impairment, and in elderly. Monitor blood pressure, kidney function (creatinine) and potassium regularly. Risk of angioedema (swelling of face/lips/tongue) especially with ACE inhibitors—seek urgent care if it occurs. Avoid potassium supplements/salt substitutes unless prescribed.

Potassium supplements or potassium-sparing diuretics (spironolactone/amiloride)
other antihypertensives and diuretics (additive hypotension)
NSAIDs (ibuprofen/diclofenac—may reduce effect and worsen kidney function)
lithium (toxicity risk)
aliskiren (avoid in diabetes/renal impairment)
trimethoprim (hyperkalemia risk)
