Important Warnings

Not recommended in pregnancy (can harm the fetus) and avoid during breastfeeding unless advised. Use caution in kidney disease, liver impairment, dehydration/low salt state, heart failure, or history of angioedema. Monitor blood pressure, kidney function and serum potassium; risk of hypotension especially after first dose in volume-depleted patients.

ACE inhibitors or aliskiren (higher risk of kidney problems/hyperkalemia; avoid especially in diabetes)
potassium supplements/potassium-sparing diuretics (spironolactone/amiloride) and salt substitutes (hyperkalemia)
NSAIDs (reduced effect and kidney risk)
diuretics (additive hypotension)
lithium (increased lithium levels/toxicity)
