Acenocoumarol is a vitamin K antagonist anticoagulant. It inhibits vitamin K–dependent clotting factor synthesis (factors II, VII, IX and X) in the liver. This reduces blood clot formation and helps prevent harmful clots from growing or forming.
Take exactly as prescribed by your doctor, usually once daily at the same time each day. Dose is individualized based on INR/PT results; do not change dose or stop abruptly without medical advice. Can be taken with or without food; maintain a consistent diet with respect to vitamin K intake and attend regular INR monitoring.

Risk of serious bleeding; seek urgent care for unusual bleeding, severe headache, weakness, vomiting blood, black stools, or after any injury/fall. Regular INR monitoring is essential. Use with caution in liver/kidney disease, uncontrolled hypertension, history of bleeding disorders, stomach/intestinal ulcers, recent surgery, and in the elderly. Many foods and medicines affect INR; keep vitamin K intake consistent and inform your doctor about all medicines and supplements. Do not use without specialist advice in patients with active bleeding.
NSAIDs/aspirin and other antiplatelets (increased bleeding), other anticoagulants (heparin, DOACs) (increased bleeding), antibiotics (e.g., metronidazole, macrolides, quinolones) (may increase INR), antifungals (fluconazole) (may increase INR), amiodarone (may increase INR), antiepileptics like phenytoin/carbamazepine (may alter INR), rifampicin (may decrease effect), thyroid hormones (may increase effect), corticosteroids (may affect INR/bleeding), herbal products like ginkgo/garlic (bleeding) and St John’s wort (reduced effect), vitamin K supplements (reduced effect)
