Type 2 diabetes mellitus (adult) to improve blood glucose control when diet and exercise alone or single therapy is inadequate
How It Works
Glimepiride (a sulfonylurea) stimulates the pancreas to release insulin and improves peripheral insulin sensitivity. Metformin reduces hepatic glucose production and decreases intestinal glucose absorption while improving insulin sensitivity.
Directions for Use
Take orally as prescribed, usually once daily with the first main meal (or as directed). Swallow whole (do not crush/chew if SR/ER). Monitor blood glucose regularly; follow diet and exercise advice.

Hypoglycaemia, nausea, vomiting, diarrhoea, abdominal pain, metallic taste, dizziness, headache, weight gain
Warnings & Precautions
Risk of hypoglycaemia (higher with missed meals, alcohol, strenuous exercise); do not skip meals. Metformin may rarely cause lactic acidosis—risk increased with kidney disease, dehydration, severe infection, alcohol use, heart/liver failure; kidney function should be assessed before and during treatment. Temporarily stop metformin around iodinated contrast procedures and major surgery as advised. Use caution in elderly and in those with G6PD deficiency. Seek urgent care for symptoms of severe hypoglycaemia or lactic acidosis (rapid breathing, severe weakness, abdominal pain, confusion).
Drug Interactions
Alcohol (increases hypoglycaemia/lactic acidosis risk), insulin or other antidiabetics (additive hypoglycaemia), beta-blockers (mask hypoglycaemia symptoms), corticosteroids and thiazide diuretics (raise glucose), ACE inhibitors (may enhance glucose-lowering), NSAIDs/salicylates (increase hypoglycaemia risk), rifampicin (reduces effect), azole antifungals and macrolides (increase sulfonylurea effect), iodinated contrast media (lactic acidosis risk with metformin), cimetidine (raises metformin levels)
