Treatment of epilepsy: adjunct therapy for partial-onset (focal) seizures with or without secondary generalization, and primary generalized tonic-clonic seizures (as prescribed by neurologist)
Perampanel is an antiepileptic that selectively blocks AMPA-type glutamate receptors in the brain. This reduces excessive excitatory neurotransmission and helps prevent the abnormal electrical activity that triggers seizures.
Take exactly as prescribed, usually once daily at bedtime; may be taken with or without food. Swallow tablet whole with water. Do not stop abruptly; dose is usually titrated gradually. If a dose is missed, take the next dose at the usual time (do not double).
Dizziness, drowsiness, fatigue, headache, nausea, irritability, aggression, mood changes, imbalance/ataxia, falls, weight gain
May cause dizziness, sleepiness and impaired coordination—avoid driving/operating machinery until effects are known. Risk of serious psychiatric/behavioral reactions (aggression, anger, hostility, suicidal thoughts); seek medical help if mood/behavior changes occur. Use caution in elderly and those at risk of falls. Do not stop suddenly due to risk of increased seizures. Use caution in hepatic impairment; dose adjustment may be required.
Enzyme-inducing antiepileptics (carbamazepine, phenytoin, oxcarbazepine) may reduce perampanel levels; valproate and levetiracetam generally have minimal effect. Alcohol and other CNS depressants increase sedation and dizziness. May reduce efficacy of hormonal contraceptives at higher doses—use additional contraception as advised.
