Treatment of acute hypocalcemia and tetany, hyperkalemia (cardiac membrane stabilization), hypermagnesemia, calcium channel blocker overdose (adjunct), calcium supplementation when oral route not possible
Calcium gluconate provides ionized calcium required for normal neuromuscular and cardiac function. In hyperkalemia it stabilizes cardiac myocyte membranes, reducing risk of arrhythmias without lowering potassium levels.
Administer only under medical supervision. Given slow IV (preferred) with ECG monitoring in indicated emergencies; avoid rapid IV push. Ensure secure IV access to prevent extravasation; dosing depends on condition and labs.
Flushing, nausea, vomiting, hypotension (with rapid infusion), bradycardia/arrhythmias, metallic/chalky taste, injection site pain, tissue irritation or necrosis with extravasation
Do not use in hypercalcemia, ventricular fibrillation, severe digitalis toxicity; use extreme caution with digoxin. Administer slowly; monitor calcium levels and ECG. Avoid IM in infants/children if possible due to tissue injury risk. Extravasation can cause severe tissue damage.
Digoxin (increased arrhythmia risk), calcium channel blockers (reduced effect), thiazide diuretics (hypercalcemia risk), ceftriaxone in neonates (avoid co-administration due to precipitation risk), tetracyclines (reduced absorption if oral given later)
