Type 2 diabetes mellitus (non-insulin-dependent diabetes) to improve blood glucose control, usually along with diet and exercise; may be used alone or with other antidiabetic medicines as prescribed.
Gliclazide is a sulfonylurea antidiabetic that stimulates insulin release from pancreatic beta cells by closing ATP-sensitive potassium channels. This increases insulin secretion and helps lower blood glucose, particularly after meals.
Oral tablet. Take exactly as prescribed, usually once daily with breakfast (do not skip meals). Swallow whole with water; do not crush or chew modified-release tablets. Monitor blood sugar regularly and follow diet/exercise advice.
Hypoglycaemia (low blood sugar), nausea, indigestion, abdominal pain, diarrhoea, constipation, headache, dizziness, weight gain, skin rash
Risk of hypoglycaemia—more likely if meals are missed, with strenuous exercise, alcohol, elderly patients, or with kidney/liver impairment. Use with caution in renal or hepatic disease and in G6PD deficiency (risk of haemolysis). Not for type 1 diabetes or diabetic ketoacidosis. Inform doctor if you have fever, infection, surgery, or stress as dose changes may be needed. Watch for signs of low blood sugar (sweating, tremor, palpitations, confusion).
Other antidiabetics/insulin (increased hypoglycaemia), fluconazole (increases gliclazide effect), miconazole (contraindicated/avoid; severe hypoglycaemia), clarithromycin (may increase effect), beta-blockers (mask hypoglycaemia symptoms), ACE inhibitors (may increase hypoglycaemia), corticosteroids and thiazide diuretics (raise blood glucose), NSAIDs and warfarin (may increase hypoglycaemia/bleeding risk—monitor), alcohol (increases hypoglycaemia risk)
