Type 2 diabetes mellitus (when diet and exercise alone or dual therapy is insufficient); helps control post‑meal and fasting blood glucose
Glimepiride increases insulin release from the pancreas and improves insulin sensitivity. Metformin reduces glucose production in the liver and improves peripheral glucose uptake. Voglibose delays intestinal carbohydrate absorption, lowering post‑meal glucose spikes.
Take orally as prescribed, usually once daily with the first main meal (or as advised). Swallow whole; do not crush/chew (metformin SR/ER). Take with food to reduce stomach upset; monitor blood glucose regularly.
Nausea, vomiting, diarrhoea, abdominal pain, flatulence, loss of appetite, headache, dizziness, hypoglycaemia, weight gain, taste disturbance
Risk of hypoglycaemia (higher with missed meals, alcohol, strenuous exercise); lactic acidosis risk with metformin (seek help for rapid breathing, severe weakness, abdominal pain). Use caution/avoid in severe kidney impairment; assess renal function before and during treatment. Avoid in severe liver disease, severe infection, dehydration, shock, or hypoxic states. Stop temporarily before/after iodinated contrast or major surgery as advised. Not for type 1 diabetes or diabetic ketoacidosis.
Alcohol (increases hypoglycaemia/lactic acidosis risk), insulin/other antidiabetics (additive hypoglycaemia), beta-blockers (mask hypoglycaemia symptoms), corticosteroids/diuretics (raise glucose), ACE inhibitors (may increase hypoglycaemia), fluoroquinolones (glucose disturbances), cimetidine (may increase metformin levels), iodinated contrast media (kidney injury/lactic acidosis risk), warfarin (possible effect changes)
