Glimepiride is a sulfonylurea that stimulates the pancreas (beta cells) to release insulin. It also helps the body use insulin more effectively, thereby lowering blood glucose levels.
Oral tablet. Take exactly as prescribed, usually once daily with breakfast or the first main meal. Swallow with water; do not skip meals after taking it. Dose adjustments are based on blood sugar; do not stop abruptly without medical advice.
Hypoglycaemia (low blood sugar), dizziness, headache, nausea, abdominal pain, diarrhoea, weight gain, skin rash
Risk of hypoglycaemia—more likely with missed meals, excessive exercise, alcohol intake, elderly, or kidney/liver impairment; carry a quick source of glucose. Use with caution in renal/hepatic disease and in patients with G6PD deficiency (risk of haemolysis). Not for type 1 diabetes or diabetic ketoacidosis. Monitor blood glucose/HbA1c regularly. Inform doctor before surgery/acute illness; dose changes may be needed. Rare but serious: severe allergic reactions, liver problems, blood count changes—seek care if jaundice, dark urine, unusual bruising/bleeding occur.
Other antidiabetics/insulin (increased hypoglycaemia), alcohol (increased risk of hypoglycaemia), beta-blockers (may mask hypoglycaemia symptoms), ACE inhibitors (may enhance glucose-lowering), corticosteroids/thiazide diuretics/sympathomimetics (may raise glucose), azole antifungals (e.g., fluconazole), macrolides (e.g., clarithromycin), quinolones, NSAIDs and salicylates (may increase hypoglycaemia), rifampicin and phenytoin (may reduce effect)
