Metformin is an insulin-sensitizing antidiabetic (biguanide). It lowers glucose production by the liver and improves peripheral uptake and utilization of glucose, helping reduce blood sugar without usually causing hypoglycaemia when used alone.
Oral sustained-release tablet. Take exactly as prescribed, usually once daily with or after the evening meal; swallow whole with water (do not crush, chew, or split). Take at the same time each day; if a dose is missed, take it when remembered unless it is near the next dose.
Nausea, diarrhoea, vomiting, abdominal pain, loss of appetite, metallic taste, flatulence, headache, weakness, vitamin B12 deficiency (long-term)
Risk of lactic acidosis (rare but serious)—seek urgent care for rapid breathing, severe weakness, unusual muscle pain, stomach pain with vomiting, or feeling very cold. Use with caution in kidney disease; dose adjustment/avoidance may be needed and renal function monitoring is recommended. Avoid in severe liver disease, severe dehydration, acute heart failure, severe infection, or conditions causing low oxygen. Temporarily stop before/after iodinated contrast imaging or major surgery as advised. Monitor blood glucose and HbA1c regularly; long-term use may reduce vitamin B12—periodic monitoring may be needed.
Alcohol (increases lactic acidosis risk), iodinated contrast media, carbonic anhydrase inhibitors (e.g., topiramate, acetazolamide), cimetidine, dolutegravir, ranolazine, diuretics and ACE inhibitors/ARBs (via effects on kidney function), corticosteroids and thyroid hormones (may raise glucose), other antidiabetics/insulin (may increase hypoglycaemia risk)
