Neuropathic (nerve) pain such as diabetic neuropathy, post-herpetic neuralgia; nerve pain associated with radiculopathy/sciatica; pain with tingling, burning, numbness.
Pregabalin reduces release of pain-signaling neurotransmitters by binding to the alpha-2-delta subunit of voltage-gated calcium channels in the nervous system. Nortriptyline increases noradrenaline/serotonin activity and modulates pain pathways, improving neuropathic pain symptoms.
Take by mouth exactly as prescribed, usually once daily at bedtime (may cause drowsiness). Can be taken with or without food; swallow whole with water. Do not stop abruptly—doctor may taper dose.
Dizziness, sleepiness/drowsiness, dry mouth, constipation, blurred vision, weight gain, swelling of legs/feet (edema), fatigue, difficulty concentrating
May cause dizziness and sleepiness—avoid driving/operating machinery until you know the effect. Use caution in elderly and in kidney disease (dose adjustment needed for pregabalin). Nortriptyline can cause constipation, urinary retention, and may worsen glaucoma/prostatic enlargement; caution in heart disease/arrhythmias. Do not stop suddenly (withdrawal symptoms). Monitor for mood changes/suicidal thoughts. Seek urgent care for facial swelling, severe rash, breathing difficulty (allergy/angioedema).
Alcohol and other CNS depressants (opioids, benzodiazepines, sedating antihistamines) increase drowsiness/respiratory depression risk; MAO inhibitors (contraindicated with nortriptyline); SSRIs/SNRIs and other serotonergic drugs (serotonin syndrome risk); other tricyclics/anticholinergics (additive side effects); QT-prolonging drugs (arrhythmia risk); strong CYP2D6 inhibitors (e.g., fluoxetine, paroxetine, quinidine) may increase nortriptyline levels.
