How It Works
Vildagliptin is a DPP-4 inhibitor that increases incretin hormones (GLP-1, GIP), enhancing glucose-dependent insulin release and reducing glucagon. Metformin reduces hepatic glucose production and improves insulin sensitivity, lowering blood glucose without causing significant hypoglycaemia when used alone.
Directions for Use
Oral tablet; take as prescribed, usually with meals to reduce stomach upset. Swallow whole with water; do not crush/chew if it is SR/ER. Take at the same time each day and monitor blood glucose as advised.
Side Effects
Nausea, diarrhoea, abdominal pain, loss of appetite, metallic taste, headache, dizziness, hypoglycaemia (especially with insulin/sulfonylureas), upper respiratory tract infection
Warnings & Precautions
Prescription only. Metformin may rarely cause lactic acidosis—risk increases with kidney impairment, dehydration, severe infection, alcohol excess, liver disease, or heart failure; stop and seek urgent care if deep/rapid breathing, severe weakness, abdominal pain, or unusual sleepiness occurs. Avoid/withhold around iodinated contrast procedures and major surgery as advised. Check renal function before and during therapy; dose adjustments/avoidance required in reduced eGFR. Monitor for pancreatitis (severe persistent abdominal pain), hepatitis/jaundice, and vitamin B12 deficiency with long-term metformin. Not for type 1 diabetes or diabetic ketoacidosis.

Alcohol (increases lactic acidosis risk), iodinated contrast media, carbonic anhydrase inhibitors (e.g., topiramate/acetazolamide), cimetidine, diuretics, corticosteroids, thyroid hormones, sympathomimetics, other antidiabetics (insulin/sulfonylureas—hypoglycaemia risk), ACE inhibitors/ARBs (may enhance glucose-lowering), CYP/P-gp significant interactions are not prominent for vildagliptin
