Treatment of anxiety disorders and panic disorder (with or without agoraphobia); short-term relief of severe anxiety symptoms when prescribed by a doctor
Alprazolam is a benzodiazepine that enhances the effect of GABA, an inhibitory neurotransmitter in the brain. This reduces excessive neuronal activity, producing anxiolytic and calming effects.
Oral sustained-release tablet. Take exactly as prescribed, usually once daily at the same time. Swallow whole; do not crush, break, or chew. Can be taken with or without food; avoid sudden discontinuation—dose should be tapered by a doctor.
Drowsiness, dizziness, fatigue, impaired coordination, slowed reflexes, confusion, memory problems, headache, dry mouth, nausea, constipation
Prescription-only benzodiazepine with risk of dependence, tolerance and withdrawal (may be severe: anxiety, insomnia, seizures) especially with long use/high doses. Avoid driving/operating machinery due to sedation. Use caution in elderly, respiratory disease (COPD/sleep apnea), liver impairment, depression/suicidal risk, history of substance misuse. Do not stop abruptly. Concomitant use with opioids, alcohol or other CNS depressants can cause profound sedation, respiratory depression, coma or death.
Alcohol and other CNS depressants (opioids, sedating antihistamines, antipsychotics) increase sedation/respiratory depression; CYP3A4 inhibitors (ketoconazole/itraconazole, macrolides like erythromycin/clarithromycin, HIV protease inhibitors, some azoles) increase alprazolam levels; CYP3A4 inducers (carbamazepine, phenytoin, rifampicin, St John’s wort) reduce effect; grapefruit juice may increase levels
