Important Warnings

Do not use in children below recommended age/weight without medical advice. Do not use if there is blood in stools, high fever, dysentery, acute ulcerative colitis, antibiotic-associated colitis (C. difficile), or bacterial enterocolitis (e.g., Salmonella/Shigella/Campylobacter). Stop and seek medical help if constipation, abdominal distension, severe abdominal pain, or no improvement within 48 hours. Use with caution in liver disease. Overdose/misuse can cause serious heart rhythm problems (QT prolongation, torsades) and CNS toxicity—do not exceed recommended dose. Ensure rehydration, especially in children and elderly.

Drug Interactions

CYP3A4/CYP2C8 inhibitors (e.g.
clarithromycin/erythromycin
ketoconazole/itraconazole
gemfibrozil) may increase loperamide levels; P-gp inhibitors (e.g.
quinidine
ritonavir) increase exposure; other QT-prolonging drugs may raise arrhythmia risk; CNS depressants may increase drowsiness.
