NSAIDs may cause stomach/intestinal bleeding or ulcers—risk higher in elderly, those with prior ulcers, alcohol use, or with steroids/anticoagulants. Can worsen kidney function and cause fluid retention; use caution in kidney disease, heart failure, dehydration, and uncontrolled hypertension. May increase risk of heart attack or stroke with long-term/high-dose use; use lowest effective dose for shortest duration. Avoid in known NSAID allergy/aspirin-sensitive asthma. Stop and seek care for black stools, vomiting blood, severe abdominal pain, chest pain, breathlessness, facial swelling, or severe rash. Avoid use after 20 weeks of pregnancy unless advised; contraindicated in last trimester.
