Important Warnings

Monitor blood pressure, kidney function and electrolytes (especially potassium, sodium, magnesium). Risk of dehydration and hypotension, especially in elderly, on low-salt diet, or with vomiting/diarrhoea. Use caution in kidney impairment, liver disease, gout (may raise uric acid), diabetes (may affect glucose), and in patients with electrolyte imbalance. Avoid use in severe renal impairment/anuria. Seek help if swelling of face/lips, breathing difficulty (angioedema), severe dizziness/fainting, or decreased urine occurs.

Other antihypertensives (additive hypotension)
NSAIDs (reduced effect and kidney risk)
lithium (increased lithium toxicity)
potassium supplements/potassium-sparing diuretics/salt substitutes (risk of high potassium with telmisartan)
diuretics or corticosteroids (electrolyte imbalance)
ACE inhibitors or aliskiren (increased renal/hyperkalaemia risk; avoid in diabetes with aliskiren)
digoxin (telmisartan may increase levels)
antidiabetic medicines (may require dose adjustment)
