Telmisartan 40 mg + Chlorthalidone 12.5 mg
Treatment of hypertension (high blood pressure), especially when single-drug therapy is inadequate; helps reduce risk of cardiovascular events related to uncontrolled BP.
Telmisartan is an angiotensin II receptor blocker (ARB) that relaxes blood vessels by blocking angiotensin II, lowering blood pressure. Chlorthalidone is a thiazide-like diuretic that increases salt and water excretion, reducing blood volume and BP. The combination provides additive BP-lowering effect.
Take as prescribed by doctor, usually once daily at the same time each day. Can be taken with or without food; swallow tablet with water. Because it contains a diuretic, taking it in the morning may reduce nighttime urination. Do not stop suddenly without medical advice.

Monitor blood pressure, kidney function and electrolytes (especially potassium, sodium, magnesium).
