Important Warnings

Contraindicated with concomitant ACE inhibitors and in patients with history of angioedema related to ACE inhibitor/ARB; maintain 36-hour gap after stopping ACE inhibitor. Avoid in pregnancy (fetal toxicity) and generally not recommended during breastfeeding. Monitor blood pressure, kidney function and serum potassium, especially in elderly, dehydration, renal impairment, diabetes, or when used with diuretics. Use caution in severe hepatic impairment; may cause angioedema—seek urgent care if swelling of face/lips/tongue or breathing difficulty occurs.

ACE inhibitors (contraindicated; 36-hour washout)
other ARBs/aliskiren (avoid; especially in diabetes)
potassium supplements/salt substitutes and potassium-sparing diuretics (hyperkalemia)
NSAIDs including COX-2 inhibitors (worsen renal function)
lithium (increased lithium levels/toxicity)
diuretics and other antihypertensives (additive hypotension)
