Important Warnings

Not for type 1 diabetes or diabetic ketoacidosis. Metformin may rarely cause lactic acidosis—risk increases with severe kidney disease, dehydration, sepsis, heavy alcohol use, liver disease, or hypoxia; renal function should be assessed before and during therapy. Stop temporarily before/after iodinated contrast procedures and major surgery as advised. Use caution in elderly and in patients with pancreatitis history (DPP-4 inhibitors have rare reports of pancreatitis); seek care for severe abdominal pain. Monitor blood glucose, HbA1c, and for vitamin B12 deficiency with long-term metformin. Do not use if severe renal impairment (eGFR <30 mL/min/1.73 m²).
