Risk of dehydration and low blood pressure—ensure adequate fluid intake. Tolvaptan can cause serious liver injury in some uses; monitor liver function if advised and stop if symptoms like fatigue, loss of appetite, right upper abdominal pain, dark urine or jaundice occur. Avoid in anuria, inability to sense/respond to thirst, hypovolemic hyponatremia, urgent need to raise sodium, or severe liver disease unless specifically directed. Use caution in kidney disease, elderly, and with diuretics.
