Important Warnings

Risk of suicidal thoughts/behaviour especially in young adults; monitor for mood/behaviour changes. Do not use with MAO inhibitors (or within 14 days) due to serotonin syndrome risk. Serotonin syndrome may occur with other serotonergic drugs; seek urgent help for agitation, fever, tremor, diarrhea. May cause abnormal bleeding especially with NSAIDs/anticoagulants, may cause hyponatremia (more in elderly/diuretics). Use caution in seizure disorder, bipolar disorder/mania, liver disease. Do not stop abruptly—taper to avoid withdrawal symptoms. May cause dizziness/somnolence; avoid driving if affected.

methylene blue
other antidepressants (SSRIs/SNRIs/TCAs)
triptans
tramadol
lithium
tryptophan
St John’s wort
dextromethorphan (serotonin syndrome risk); NSAIDs/aspirin/clopidogrel/warfarin (bleeding risk); strong CYP3A4 inhibitors (e.g.
ketoconazole
clarithromycin
ritonavir) may increase levels; CYP3A4 inducers (e.g.
carbamazepine
rifampicin
phenytoin) may decrease effect.
