Drowsiness, dizziness, fatigue, impaired coordination, memory problems, confusion, slurred speech, headache, dry mouth, nausea, constipation
May cause sedation, impaired alertness and falls—avoid driving or operating machinery. Risk of dependence, misuse and withdrawal; use the lowest effective dose for the shortest duration. Do not stop abruptly (may cause withdrawal symptoms, rebound anxiety, seizures). Use caution in elderly, depression/suicidal thoughts, history of substance abuse, liver disease, kidney disease, respiratory disease (COPD/sleep apnea). Concomitant use with opioids or other CNS depressants can cause profound sedation, respiratory depression, coma or death.
Alcohol and other CNS depressants (opioids, sedating antihistamines, antipsychotics) increase sedation/respiratory depression; CYP3A4 inhibitors (ketoconazole, itraconazole, clarithromycin/erythromycin, ritonavir, cobicistat) increase alprazolam levels; CYP3A4 inducers (carbamazepine, phenytoin, rifampicin, St John’s wort) may reduce effect; grapefruit juice may increase levels
