Azathioprine is an immunosuppressant. It is converted in the body to metabolites that inhibit purine synthesis, reducing DNA/RNA formation in rapidly dividing immune cells. This decreases immune response and inflammation.

May cause serious bone marrow suppression and infections; seek help for fever, mouth ulcers, unexplained bruising/bleeding. Regular monitoring of CBC and liver function is essential. Increased risk of certain cancers (e.g., lymphoma, skin cancer) with long-term use—use sun protection. Use caution in liver disease and in patients with low/absent TPMT or NUDT15 activity (higher toxicity risk). Avoid live vaccines during therapy. Stop and consult doctor if severe abdominal pain (pancreatitis) or jaundice occurs.
Drug Interactions
Allopurinol, febuxostat (markedly increase azathioprine toxicity; avoid febuxostat and reduce dose with allopurinol as directed), ACE inhibitors (risk of anaemia/leukopenia), cotrimoxazole/trimethoprim (bone marrow suppression), warfarin (reduced anticoagulant effect), aminosalicylates such as mesalamine/sulfasalazine (increased myelosuppression risk), other immunosuppressants/biologics (additive infection risk), live vaccines
