Neuropathic pain (nerve pain) such as diabetic neuropathy, post-herpetic neuralgia, radiculopathy/sciatica-related nerve pain; adjunct for chronic neuropathic pain conditions as advised by doctor
Gabapentin reduces abnormal nerve signalling by modulating calcium channels in the nervous system, decreasing pain transmission. Nortriptyline (a tricyclic antidepressant) increases levels of norepinephrine and serotonin in the spinal cord/brain, enhancing descending pain inhibition and improving neuropathic pain symptoms.
Tablet; take exactly as prescribed. Usually taken once daily at night or as directed; can be taken with or without food. Swallow whole with water; do not stop suddenly—dose may need gradual tapering.
Drowsiness, dizziness, fatigue, dry mouth, constipation, blurred vision, weight gain, nausea, unsteadiness, swelling of feet/ankles
May cause significant drowsiness/dizziness—avoid driving or operating machinery until you know the effect. Use with caution in elderly, history of depression/suicidal thoughts, heart disease/arrhythmias, glaucoma, urinary retention/prostate enlargement, and kidney impairment (gabapentin dose adjustment may be needed). Do not stop abruptly; withdrawal or symptom worsening may occur. Seek urgent care for severe allergic reaction, breathing difficulty, severe rash, or mood/behavior changes.
Alcohol and other CNS depressants (additive sedation), opioids (increased sedation/respiratory depression risk), antacids containing aluminium/magnesium (reduce gabapentin absorption—separate by 2 hours), MAO inhibitors (contraindicated/avoid with nortriptyline), other antidepressants/serotonergic drugs (serotonin syndrome risk), QT-prolonging drugs and antiarrhythmics (arrhythmia risk), antihistamines and benzodiazepines (additive drowsiness)
Store below 25°C in a cool, dry place away from moisture and direct light. Keep in original blister pack and out of reach of children.
