dizziness, headache, fatigue, nausea, back pain, diarrhea, low blood pressure, increased potassium
Not recommended in pregnancy (can harm fetus) and generally avoid during breastfeeding unless advised. Use caution in kidney disease, renal artery stenosis, liver impairment, dehydration/low salt states, and in elderly due to risk of low blood pressure. Monitor blood pressure, kidney function (serum creatinine) and potassium; risk of hyperkalemia especially with kidney disease or potassium-raising drugs. Seek urgent care for facial/lip swelling (angioedema) though rare.
ACE inhibitors (e.g., enalapril), aliskiren (especially in diabetes/kidney impairment), potassium supplements/potassium-sparing diuretics (spironolactone, amiloride), NSAIDs (ibuprofen, diclofenac) may reduce effect and worsen kidney function, lithium (toxicity risk), diuretics may increase hypotension risk
