Treatment of essential hypertension (high blood pressure), especially when blood pressure is not adequately controlled with telmisartan alone; helps reduce risk of cardiovascular complications related to uncontrolled hypertension.
Telmisartan is an angiotensin II receptor blocker (ARB) that relaxes blood vessels by blocking angiotensin II, lowering blood pressure. Hydrochlorothiazide is a thiazide diuretic that increases salt and water excretion via kidneys, reducing fluid volume and blood pressure. The combination provides additive BP-lowering effect.
Take as prescribed by a doctor, usually once daily at the same time each day. Can be taken with or without food; swallow tablet with water. Because it contains a diuretic, taking it in the morning is often preferred to reduce nighttime urination; do not stop suddenly without medical advice.
Dizziness, low blood pressure, increased urination, headache, fatigue, nausea, muscle cramps, increased blood urea/creatinine, electrolyte imbalance (low potassium/low sodium), increased uric acid
Not recommended in pregnancy (can harm fetus); avoid if allergic to telmisartan, hydrochlorothiazide or sulfonamides. Use caution in kidney or liver disease, dehydration, low blood pressure, heart failure, renal artery stenosis, diabetes, gout, and in elderly. Monitor blood pressure, kidney function and electrolytes periodically; risk of dehydration and electrolyte disturbances—drink adequate fluids unless advised otherwise. May cause dizziness—avoid driving/operating machinery until effects are known. Hydrochlorothiazide may increase blood sugar and uric acid; telmisartan may increase potassium (risk higher with kidney disease or potassium-sparing drugs).
Potassium supplements/salt substitutes, potassium-sparing diuretics (spironolactone/eplerenone/amiloride), ACE inhibitors or aliskiren (especially in diabetes/kidney disease), NSAIDs (ibuprofen/diclofenac) may reduce effect and worsen kidney function, lithium (increased toxicity), other antihypertensives (additive hypotension), corticosteroids and laxatives (worsen low potassium), antidiabetic medicines (dose adjustment may be needed), alcohol (increases dizziness)
Store below 30°C in a cool, dry place; protect from moisture and direct light. Keep in original blister/pack and out of reach of children.
