Type 2 diabetes mellitus (as add-on to diet and exercise) to improve post-meal and overall blood glucose control
Voglibose is an alpha-glucosidase inhibitor that delays digestion and absorption of carbohydrates in the intestine, reducing post-meal blood sugar spikes. Metformin lowers glucose production in the liver and improves insulin sensitivity, helping the body use glucose more effectively.
Oral tablets. Take exactly as prescribed. Voglibose-containing combinations are usually taken just before meals/with the first bite of food to reduce postprandial glucose; metformin is typically taken with meals to reduce stomach upset. Swallow whole with water.
Flatulence, abdominal discomfort, diarrhea, nausea, vomiting, loss of appetite, metallic taste, headache, hypoglycemia (when used with other antidiabetics), skin rash
Not for type 1 diabetes or diabetic ketoacidosis. Metformin: risk of lactic acidosis (rare but serious)—use caution in kidney or liver impairment, dehydration, severe infection, hypoxia, heart failure, and in the elderly; stop temporarily before/after iodinated contrast procedures and major surgery as advised. Voglibose: may cause GI side effects; avoid/use caution in inflammatory bowel disease, intestinal obstruction/ulceration, or chronic intestinal disorders. Monitor blood glucose, HbA1c, renal function; long-term metformin can cause vitamin B12 deficiency. If hypoglycemia occurs (especially with insulin/sulfonylureas), treat with glucose (not sucrose).
Alcohol (increases lactic acidosis risk with metformin), iodinated contrast media, carbonic anhydrase inhibitors (e.g., topiramate, acetazolamide), cimetidine, corticosteroids, thiazide/loop diuretics, beta-agonists, thyroid hormones, insulin and sulfonylureas (increased hypoglycemia risk), digoxin
