Treatment and prevention of gout (chronic hyperuricaemia), prevention of uric acid kidney stones, prevention/treatment of high uric acid due to cancer chemotherapy (tumour lysis syndrome)
Allopurinol inhibits xanthine oxidase, an enzyme required for conversion of hypoxanthine and xanthine to uric acid. This lowers serum and urinary uric acid levels and helps prevent urate crystal formation and deposits in joints/kidneys.
Tablet for oral use. Take exactly as prescribed, preferably after food and with plenty of water. Continue long term; do not stop abruptly. Dose may be started low and titrated; ensure good hydration. If a dose is missed, take it when remembered unless it is close to the next dose.
Skin rash, nausea, vomiting, diarrhoea, abdominal pain, headache, dizziness, drowsiness, taste changes, elevated liver enzymes
Do not use if allergic to allopurinol. Stop and seek urgent medical help if rash, fever, sore throat, mouth ulcers, facial swelling, or peeling skin occurs (risk of severe cutaneous adverse reactions including SJS/TEN/DRESS). Use with caution in kidney or liver disease; dose adjustment may be required and monitoring advised. May precipitate an acute gout attack when started—doctor may prescribe prophylaxis (e.g., colchicine/NSAID). Maintain adequate fluid intake; monitor uric acid. Risk of hypersensitivity is higher in renal impairment and with thiazide diuretics; HLA-B*58:01 testing may be considered in high-risk ethnic groups.
Azathioprine, mercaptopurine (require major dose reduction/avoid), warfarin (may increase effect), theophylline, didanosine, cyclophosphamide/chlorpropamide (increased toxicity risk), ampicillin/amoxicillin (increased rash risk), thiazide diuretics (increased hypersensitivity risk), ACE inhibitors (reported increased hypersensitivity), probenecid/uricosurics (may alter levels)
