Important Warnings

Prescription-only antidiabetic. Risk of hypoglycaemia (higher with missed meals, alcohol, strenuous exercise, elderly, or with other antidiabetics). Metformin can rarely cause lactic acidosis—seek urgent care for rapid breathing, severe weakness, abdominal pain, vomiting, or unusual sleepiness. Avoid/use with caution in severe kidney impairment; monitor renal function before and during therapy. Use caution in liver disease, heart failure, dehydration, severe infection, or low oxygen states. Temporarily stop metformin before/after iodinated contrast procedures or major surgery as advised by doctor. Monitor blood glucose and HbA1c regularly.

ACE inhibitors (may increase hypoglycaemia risk)
quinolones (may alter glucose)
cimetidine (may increase metformin levels)
iodinated contrast media (risk of kidney injury/lactic acidosis)
NSAIDs and other highly protein-bound drugs (may increase sulfonylurea effect)
