Important Warnings

Do not stop suddenly (risk of worsening angina/heart attack); dose should be tapered. Use with caution in asthma/COPD, diabetes (may mask hypoglycaemia symptoms), thyroid disorders (may mask hyperthyroidism), peripheral vascular disease, and severe renal/hepatic impairment. Contraindicated in severe bradycardia, second/third-degree AV block, cardiogenic shock, decompensated heart failure, and severe hypotension unless specifically directed. Monitor BP and pulse; seek urgent care for fainting, wheezing, or very slow pulse.

calcium channel blockers like verapamil/diltiazem (risk of bradycardia/AV block)
digoxin (bradycardia)
antiarrhythmics e.g. amiodarone (conduction issues)
clonidine (rebound hypertension if stopped abruptly)
NSAIDs (may reduce antihypertensive effect)
CYP2D6 inhibitors like fluoxetine/paroxetine/quinidine (increase metoprolol levels)
insulin/oral antidiabetics (masked hypoglycaemia)
anaesthetics (enhanced hypotension/bradycardia)
