Telmisartan 40 mg + Hydrochlorothiazide 12.5 mg (per tablet)
Treatment of hypertension (high blood pressure), especially when blood pressure is not adequately controlled with telmisartan alone; helps reduce risk of cardiovascular events associated with uncontrolled hypertension.
Telmisartan is an angiotensin II receptor blocker (ARB) that relaxes blood vessels by blocking angiotensin II, thereby lowering blood pressure. Hydrochlorothiazide is a thiazide diuretic that helps the body remove excess salt and water through urine, further reducing blood pressure and swelling.
Tablet for oral use. Take once daily at the same time each day, with or without food, as prescribed by the doctor. Swallow whole with water; do not stop abruptly without medical advice. Because it contains a diuretic, taking it in the morning may help avoid night-time urination.

Not recommended in pregnancy (may harm the unborn baby) especially in 2nd/3rd trimester. Use caution in kidney or liver disease, dehydration, low blood pressure, heart failure, or electrolyte imbalance. Monitor blood pressure, kidney function (creatinine), and electrolytes (potassium/sodium). May cause dizziness—avoid driving if affected. Use caution in diabetes (may affect glucose) and gout (may raise uric acid). Avoid salt substitutes or supplements containing potassium unless advised.
