Important Warnings

Risk of hypoglycaemia (especially with missed meals, heavy exercise, alcohol, elderly, kidney/liver impairment); metformin may rarely cause lactic acidosis—seek urgent care for rapid breathing, severe weakness, abdominal pain, or unusual sleepiness. Use caution/avoid in severe renal impairment; kidney function should be checked before and during therapy. Temporarily stop metformin before/after iodinated contrast procedures and during severe dehydration, serious infection, surgery, or hypoxia. Use with caution in hepatic disease; monitor for allergic reactions or blood disorders (rare).

Alcohol (increases risk of hypoglycaemia/lactic acidosis)
insulin or other antidiabetics (additive hypoglycaemia)
beta-blockers (mask hypoglycaemia symptoms)
ACE inhibitors (may enhance glucose-lowering)
corticosteroids/thiazide diuretics/sympathomimetics (raise glucose)
fluoroquinolones (glucose disturbances)
azole antifungals and sulfonamides (may increase glimepiride effect)
rifampicin (may reduce glimepiride effect)
iodinated contrast media (lactic acidosis risk with metformin)
cimetidine (may increase metformin levels)
