Type 2 diabetes mellitus: to improve blood sugar control in adults when diet and exercise alone (or single medicine) are not enough.
Glimepiride (a sulfonylurea) increases insulin release from the pancreas and helps the body use insulin better. Metformin (a biguanide) decreases glucose production in the liver and reduces glucose absorption while improving insulin sensitivity.
Take by mouth exactly as prescribed, usually once daily with/after the first main meal to reduce stomach upset. Swallow the tablet whole (do not crush/chew if SR). Monitor blood glucose regularly and follow diet/exercise advice.

Risk of hypoglycaemia (especially with missed meals, heavy exercise, alcohol, elderly, kidney/liver impairment); metformin may rarely cause lactic acidosis—seek urgent care for rapid breathing, severe weakness, abdominal pain, or unusual sleepiness. Use caution/avoid in severe renal impairment; kidney function should be checked before and during therapy. Temporarily stop metformin before/after iodinated contrast procedures and during severe dehydration, serious infection, surgery, or hypoxia. Use with caution in hepatic disease; monitor for allergic reactions or blood disorders (rare).
