Propranolol is a non-selective beta-blocker that blocks beta-1 and beta-2 receptors. It reduces heart rate and force of contraction and lowers blood pressure by decreasing cardiac output and suppressing renin release, thereby reducing workload on the heart.
Take as prescribed. Swallow the capsule whole; do not crush or chew (TR/extended release). Usually taken once daily at the same time each day, with or without food; take consistently with respect to meals.
Fatigue, dizziness, slow heart rate (bradycardia), cold hands/feet, nausea, diarrhoea, sleep disturbances/vivid dreams, depression, shortness of breath (especially in susceptible patients)
Do not stop suddenly; may worsen angina or cause rebound hypertension—taper under medical supervision. Use with caution in asthma/COPD (may cause bronchospasm), diabetes (can mask hypoglycaemia symptoms), thyroid disorders (masks hyperthyroidism signs), peripheral vascular disease, and in patients with bradycardia, heart block, or heart failure unless advised by a cardiologist. Monitor pulse and blood pressure. Inform doctor before surgery/anaesthesia.
Verapamil/diltiazem (risk of severe bradycardia/heart block), amiodarone/other antiarrhythmics, digoxin, clonidine (rebound risk on withdrawal), insulin/oral antidiabetics (masked hypoglycaemia), NSAIDs (may reduce antihypertensive effect), other antihypertensives, alcohol (may increase dizziness), CYP inhibitors like cimetidine/fluoxetine (may increase propranolol levels), bronchodilators like salbutamol (reduced effect)
Store below 25°C in a dry place, protected from moisture and light. Keep out of reach of children.
